Change Your Name
The process to change your name on your passport varies depending on your situation. Please see the chart below to find the process that applies to you.
If you are requesting the change less than one year after your passport was issued, you should:
- Make an ACS appointment;
- Complete DS-5504 Form online and bring it for the interview;
- Bring your current U.S. Passport (original and a copy of front page);
- Submit your original or certified name change document, such as a marriage certificate, divorce decree or court order (no photocopies or notarized copies);
- Bring one passport photo;
- Please note: You will not have to submit any fees.
If you are requesting the change more than one year after your passport was issued, you should:
- Make an ACS appointment;
- Complete DS-82 Form online and bring it for the interview;
- Bring your current U.S. Passport (original and photocopy of front page);
- Submit your original or certified name change document, such as a marriage certificate, divorce decree or court order (no photocopies or notarized copies);
- Bring one passport photo;
- Pay the appropriate passport services fee.
Correct a Data or Printing Error
A passport issued with a data error (e.g., name, sex, or place of birth) or printing error (e.g., data is missing on the biographical page, discoloration, crooked printing, etc.) can be corrected at no charge if the passport is still valid.
- Make an ACS appointment;
- Complete DS-5504 Form online and bring it for the interview;
- Bring your current U.S. Passport (original and a copy of front page);
- Submit documentation showing that the information in the passport is incorrect (e.g. original birth certificate, marriage certificate, government-issued ID)
- Bring one passport photo;
- Please note:
- if Reported within one year of issue date, then The new passport will be valid for 10 years from the date it is issued.
- if Reported after one year of issue date, then The new passport will be valid until the expiration of the original, incorrect passport.
All foreign documents must be accompanied by an English translation.
