To apply for a second valid passport in person, you will need:

  1. Make ACS appointment;
  2. Complete DS-82 form online and bring it for the interview;
  3. Complete DS-4085 form online and bring it for the interview;
  4. Complete an affidavit (PDF 53 KB) attesting to the use of the second passport and bring it for the interview;
  5. Bring proof of U.S. citizenship (original and a copy): either your current valid passport or current second valid passport (in you already have one);
  6. Bring one photo;
  7. Pay the appropriate passport services fee.
